I like to imagine that he's giving the finger to some PETA activist who tried to grab Bush's dog as part of some plot to gain publicity for PETA's own twisted agenda.
What is the basis for calling the picture a fake? I can't see where it is doctored, but I'm suffering from serious sleep deprivation.
Even if it were real, I don't think he was intentionally giving anyone the finger. It's more plausible that, when hundreds of cameras are clicking away at you every time you turn around, one of them is bound to come up with a photo where it looks like you're giving someone the bird, picking your nose, or are playing pocket pool.
Former South Korean President Kim Youngsam used to do this up-and-down wave that, when caught as a still photo, looked like he was doing a Nazi salute. (I'm trying to find a link but can't right now; sleep deprivation is still my excuse).
The middle and index finger look photoshoped. Its pretty clear if you look closely (and have had some sleep). The video of him giving the finger was different, real, and classic.

Colorize me skeptical. I see what you're saying, but I'm not sure if I fully accept that explanation.
Yes, his middle finger looks photoshopped, but so do other parts of the photo that are not in question, like his right cheek, if you think about it.
If he were pointing with his index finger and that picture was photoshopped to make it look like he was giving someone the finger, the thumb would likely be in a different position than it appears in the picture.
However, if he were sticking out his finger in a gun-like gesture, the thumb might be in that position, but probably a little more erect.
Unless someone has come clean on having photoshopped this, I think that a more likely explanation is that, as I mentioned before, this is just a coincidental confluence of hand gestures that would eventually be caught if you had hundreds of cameras recording your every move during the day.
Besides these clips, there's a very real video clip of Bush giving the cameraman a playful middle finger while doing a sound check and straightening his hair. It's back from his gubernatorial days, but it's the sort of clip you just can't make up. I wish I knew where to find it, though... it's available online but I'm not sure when or where I saw it.
I think its clear the original photo would be W "beckening" with his index and middle fingers. Its one of his common gestures. I've also seen several shots of him handing Barney to Secret Service agents before getting on the chopper. The index finger was removed by cloning a chunk the right breast of his suit.
